Take a look at mouse sensitivity settings, I remember it was a factor for me.

It can be because of:
- Darklight implant (meant to reveal infiltrators when active)
- coincidence (they did not actually see you)
- graphics card/settings (it's sometimes buggy)
- hacks/cheating (worst case scenario fair-play wise)
